Instructions

Option 1: If you have been given a file number from your application, please fill in:

  • 'File number' field:
  • 'Date of birth' field.

Option 2: If you do not have a file number, please fill in:

  • 'Family name' and 'Given name' fields (if your name consists of only one word, please enter your name in both Family and Given name fields as indicated on your application form),
  • 'Date of birth' field (for example, 02 03 1977), and
  • 'Country of Citizenship' dropdown.
